Teddy requires action slips on CH documents By Aledel A. Gonzales
MANDAUE City Hall officials and employees will no longer have an excuse if the processing of papers is delayed.
With the attachment of a document routing and action slip, one can determine how long a document stayed in an office until it was endorsed to another.
Mayor Thadeo Ouano issued a memorandum stressing the need to attach the slip so the policy on the five-day approval and action on matters regarding public service is implemented.
The slip will contain the title of the document and the date it was prepared, name of the person who received it, when it was accepted and space for remarks.
The date it was endorsed to another office and the employee who processed it should also be indicated.
All the employees who processed the document should sign their names and the dates they received the document, for future reference.
After the processing of the document, a copy of the routing and action slip will be submitted to the office of the city administrator so the processing system will be properly monitored and evaluated.
