PHILHEALTH 7 will reinstate the submission of supporting documents in registering employees effective Sept. 1.
Employees registering for the first time must submit a clear copy of a birth or baptismal certificate, Government Service Insurance System/Social Security System member’s identification card, passport or any valid identification or document.
They must also submit two properly filled out registration forms (M1a Form) and the employee-members report form (ER-2) to be filled out by the employer.
Individuals who have previously registered under a different employer or a different membership category must accomplish two member data amendment forms (M2 Form) to update their records and submit two ER-2 Forms signed by the employer.
Processing
After applications for registration are processed, employees will be issued a member data record and PhilHealth number card reflecting their permanent and unique PhilHealth identification number.
Employers registering for the first time must accomplish the employer data record form (ER-1) and submit a clear copy of their Securities and Exchange Commission registration (for corporations), Department of Trade and Industry registration (for single proprietorships) or license to operate as the case may be.
Law offices, households (as employers) and other entities similarly situated may register without the supporting documents to the ER-1 Form.
As proof of registration, employers will be issued an employer data record and a certificate of registration reflecting their PhilHealth employer number.
