Meditalk: Fish oil supplements and the heart (part 2)

(Foreword: This is our continuation of our response to Jay Zee of Ma-a on his question on fish supplements and its benefits to the heart.)

DEAR Jay Zee of Ma-a,

So, let us continue and answer your raging question now. Should you take them?

I am sorry to tell you that studies have shown that the benefit to low-dose (1 gram per day) fish oil supplements is likely small. It is better for you to eat fatty fish such as sardines, herring, albacore tuna, and salmon, among others to get omega-3 fatty acids. The American Heart Association recommends fish (particularly fatty fish) at least twice a week.

If you want to lower your high triglycerides, take a high dose (4 grams per day) of omega-3. But, should you decide to take an omega-3 supplement, look for a high-quality supplement made from a reputable company.

But, please be aware that omega-3 supplements are not free from risk, as there is a small increase in bleeding. This is the reason why I don't give this to my patients who are on blood thinners like aspirin, clopidogrel or warfarin. Moreover, in the REDUCE-IT trial, it was shown that there was an increase in Afib with mega-3 supplements. This is atrial fibrillation or quivering or irregularity in heartbeats that can lead to blood clots, stroke, heart failure and other heart-related complications.

So, would you take it or not? It is a complex story and not yet complete. You have to see your physician to see if this is right for you.
