THE Davao City government launched over the weekend an online vaccination appointment scheduler and online swab certification.
In the afternoon of Friday, August 6, 2021, the Davao City Information Office announced that the Davao City Covid-19 website has opened a more convenient contactless scheme for the swabees to immediately get their results and have their certificates printed without going to the swab test centers.
To request their swab certificate, SafeDavao QR (DQR) Code holders must log in using their DQR code, birthday, and mobile phone number. The uploaded swab results will be based on the list posted by the molecular laboratory.
Meanwhile, the city government launched on Saturday, August 7, the online vaccination appointment scheduler via DQR, which allows those in priority groups to make an appointment to the vaccination sites at their convenient day and time.
For the first step, the registrant must go to profiles.safe-davao.com or click Register for Vaccination button at covid19.davaocity.gov.ph/.
For the second step, those with pre-existing DQR registration may log in as an individual using their DQR number, mobile number, and birthday. Those who do not have a DQR number may register.
For the third step, the registrant must click register and must read and understand the privacy consent terms.
“They have to make sure that all the information they entered are correct. Informed and agreeable to these terms, they may click the agree and then proceed to the next button,” the city government said.
The next button will lead them to a disclaimer, which says that only Priority A1 (Health Care Workers), Priority A2 (Senior Citizens), Priority A3 (Persons with Comorbidities), and Priority A4 (essential workers), and Priority A5 (Poor Population) can avail of the vaccination as of the moment based on the guidelines set by the World Health Organization and the Department of Health.
“Those who seek to be vaccinated are required to honestly select their priority group. Registrants are also warned that verification will be done onsite and those who are not part of the current priority groups being served shall be sent home,” the city said.
If applicable, registrants will be required to enter their company name and occupation. Each of the registrants is required to state if they suffer from allergies or that they have comorbidity or other diseases based on the provided list. Once all information is verified, they may click and submit it.
The fourth step is the scheduling proper. The registrant, after pre-registration, can now immediately check and choose the available vaccination site and the available date and time.
The fifth step is the confirmation, where details of the schedule will be shown for final review and submission.
Finally, for the sixth step, the schedule will appear at the topmost portion of the person's profile and he/she may screenshot it to present it to the vaccination site.
Reschedule and cancel options are also available. If either of the two were selected the registrant should provide a reason for the rescheduling or the canceling of the schedule.
“Note that one may only cancel or reschedule twice every week. If a person shall exceed this limit, his/her account may be temporarily blocked from choosing a schedule,” it said.
For the seventh step, a text confirmation will be sent showing the schedule which will be used to present to the vaccination site on the day of the schedule.
Step eight, additional required documents should be brought by the registrant to the vaccination sites on the day of the vaccination depending on the priority group.
For Priority A1- photocopy of proof of employment to a health facility; Priority A2- government-issued ID with a birthdate that indicates an age of 60 and above; Priority A3- photocopy of proof of comorbidity and a government-issued ID with birthdate indicating age of 18 and above, for Priority A4- photocopy or any proof of employment; and Priority A5 - Pantawid Pamilyang Pilipino Program (4Ps) ID and verification from Department of Social Welfare and Development.
The city government said the Covid-19 website will serve as the site where Dabawenyos can request Covid-19 swab tests and vaccination certificates, and vaccination schedules.
The Covid-19 website (https://covid19.davaocity.gov.ph/) launched on July 21, 2021, is a One-Stop-Shop for all Covid-19 related information, including local and national guidelines, information, and other Davao City government response against the evolving Covid-19.
As a site exclusively for Covid-19 information, it now contains all there is to know about Covid-19 and has added frequently asked questions and issuances such as local issuances and national guidelines set by the Covid-19 Inter-Agency Task Force for the Management of Emerging Infectious Diseases.
The site’s “chatbot feature” answers the frequently asked questions regarding vaccination, travel requirements, and other concerns related to Covid-19. The content will regularly be updated to address the needs of Davao constituents.
Davao City Mayor Sara Duterte-Carpio previously mentioned that the website will hopefully address concerns of the people immediately, aside from the available hotline numbers which are sometimes cannot be reached due to the multiplicity of calls and messages it caters to per day.
Covid-19 Task Force spokesperson Dr. Michelle Schlosser said the city has conducted pilot testing for the app and website to ensure a smooth operation process. (With reports from Davao CIO)
