THE Project Balik Buhay (PBB), under its PBB Junior program, will start on November 9 its vaccination rollout for kids aged 12 to 17 who are dependents of members of PBB-registered companies or organizations.
Visayas Covid-19 Vaccination Operations Center (VVOC) spokesperson Dr. Mary Jean Loreche announced this in a daily briefer held on Friday, November 5.
For individuals who want to register under the PBB Junior program, the following procedures have to be followed:
* Download the pre-listing requirements on https://biy.ly/PBBJunior-PreListing.
* Email the completed files to registration@pbbjunior.com.ph
Additional documentary requirements and consent or assent forms will be emailed upon confirmation of complete submission.
Aside from these, there are also required vaccination documents that the vaccinees will need to bring during the actual vaccination.
For minors with comorbidities, they must secure a medical certificate duly signed by the vaccinee's attending physician or pediatrician.
For minors accompanied by parents, a Philippine Statistics Authority (PSA)-issued birth certificate is required.
In absence of PSA-issued birth certificate, the parents may bring with them secondary documents like authenticated medical certificate, baptismal certificate, and school ID.
For minors accompanied by legal or judicial guardian, they should bring affidavit of guardianship executed by the guardian.
Vaccinees and guardians are also advised to bring documents with photo of the parent/guardian and the vaccine recipient to verify the documents presented.
Based on the PBB Junior vaccination application summary from October 13 to November 4, there are around 4,492 registrants in the cities of Cebu, Mandaue, Lapu-Lapu, Talisay, and Naga, as well as in the towns of Consolacion and Minglanilla.
The VVOC said the actual pediatric vaccination under the PBB Junior will be held at the IEC Convention Center on Cardinal Rosales Avenue, Cebu City.
Aside from the PBB Junior, the registration for PBB Plus will also start as soon as the final guidelines are released.
The PBB Plus is program for administering booster shots for Priority Group A or medical frontliners who have received their vaccine shots through PBB.
