Sesante: Pregnancy acne kills confidence

Dear Dr. Dana,

I’ve just discovered that I’m two months pregnant. My concern is that I have pimples. I’m worried because I am aware that a pregnant woman should not use any facial treatment.

So, what should I do? I’m not feeling good about myself right now. It’s making me lose my confidence.

Jazz

Dear Jazz,

During early pregnancy, predominance of the androgen hormone can prompt the sebaceous glands in your skin to enlarge. This increases the production of sebum, an oily substance, and coupled with blocked pores brought about by shed skin cells lining the hair follicles, create an environment for bacterial growth.

This eventually leads to infections and that skin eruption called acne. And pregnancy can sometimes trigger or worsen acne. Generally, the topical route rather than oral intake is recommended because topical creams and gels pose less exposure to the fetus. As always, when contemplating use of medications during pregnancy, the potential risks must be weighed against the benefits.

Topical treatments that can be used in pregnancy include benzoyl peroxide and azelaic acid. These topical preparations both have antibacterial effects and induce the skin to peel. For severe cases of acne, your dermatologist may safely prescribe oral erythromycin during pregnancy. This medication crosses the placenta minimally and is quickly metabolized by the body. Although less often used.

To prevent acne eruptions; wash face gently without scrubbing, using only mild soap and cleanser. Pat your skin gently to dry. Rubbing can worsen acne. Never pop or squeeze your pimples to avoid scarring. It is best to always consult your obstetrician and dermatologist before using any medication for the face. A number of over-the-counter products may help clear up acne, but may have harmful effects on the baby.

Very truly yours,

Dr. Dana R. Sesante
