Angeles City offers free cataract operation

THE Angeles City Government is offering local residents free cataract operation from October 25 to November 5, 2021.

The "Aslag ning Pagasa — Free Cataract Operation" is a joint project of Mayor Carmelo “Pogi” Lazatin Jr. and Pampanga First District Representative Carmelo “Jon” Lazatin II.

Forms for free cataract operation will be provided in the six Rural Health Units and 28 sub-health centers from October 25 to November 5, 2021.

All Angeleño residents who are interested to avail of the program should fill out the form and submit the necessary requirements to the staff of the RHUs or sub-health centers.

The needed documents are valid IDs with city address, Certificate of Barangay Indigency with the same address indicated in the valid IDs, and social case study or endorsement from the City Social Welfare and

Development Office.
